Research Top Local Specialists
Finally, I then did some research to find out who was doing robotic-assisted surgery in the local area. Similarly, I researched to see who was considered the top radiation oncologist was with experience with brachytherapy in prostate cancer.
To summarize my personal impressions:
§ Watchful waiting does not seem to be recommended for younger, healthier men who can tolerate treatment.
§ It’s not clear that surgery is more effective than radiotherapy at least in the short term. What might happen after 12 years is an unknown question.
§ The side effect profile for radiotherapy seems better initially.
§ If radiotherapy is chosen there is an increased risk for the development of a second cancer later in life.
§ If the surgery route is contemplated then both the laparoscopic and robotic-assisted procedures should be looked into in addition to the open procedure.
§ The experience of the surgeon is critical to the outcome of both the laparoscopic and robotic-assisted procedure.
Summary
After all this research I would have a long discussion with the hypothetical patient describing my findings and getting his input. Obviously, he would be getting input as well from his treating physicians.
At the end of the day, my goal would to make sure that the patient has all the information he needs to feel comfortable making his treatment decision.
